Changes

(Name, Address, Birth, Adoption, Death etc.)

You are required to report any changes pertinent to your Medicare coverage immediately by either:

  • contacting 1-888-762-8600, or
  • visiting your local SNB office,
  • completing the Medicare Change form,
  • and providing the following information, depending on the type of change.
Type of Change Information Required along with
Identifying Information

Address changes Individual name(s) affected by address change.
Incorrect spelling of name(s)/change of name A copy of birth certificate or legal name change document.
Incorrect date of birth If not born in New Brunswick, a copy of birth certificate.
For those born in New Brunswick, date of birth as indicated on birth certificate.
Marriage (or common law and want to be registered as household on Medicare Files)
  • Identifying information of both individuals;
  • Current address;
  • Family name(s) chosen at time of marriage.
  • If dependents are involved:
    • provide identifying information for each;
    • if residing other than with the applicant, provide identifying information on parent/guardian with whom they reside.
Divorce or separations
  • Former spouse's address (if unknown, please indicate "Unknown");
  • Identifying information on whom the dependents are residing with.
Custodial changes
If under the age of 16:
Copy of custody papers along with a letter containing identifying information for all affected members.
Over the age of 16 and under 19:
Letter signed by both the parent and the child indicating the arrangements made along with Identifying information.
Birth Full name of child, date of birth and sex. In the case of same-sex multiple births, please indicate which baby was born first, second etc.
Adoptions
  • From within Canada:
    • Medicare application for registration form;
    • Proof of adoption ie:
      • Adoption order;
      • Letter from adoption agency;
      • Letter from a solicitor; or
      • Letter from social worker
    • Name of child;
    • Sex;
    • Date of birth;
    • Name of social worker (if applicable);
    • Date of adoption;
    • Adoptive parents identifying information.
  • From outside Canada
    • Medicare application for registration form;
    • Copy of child's immigration document;
    • Proof of adoption ie:
      • Adoption order;
      • Letter from adoption agency; or
      • Letter from a solicitor.
    • Letter from social worker;
    • Adoptive parents identifying information.
Deaths
  • Name of deceased.
  • Date of death.
Moving out-of-province/country
  • Address in new province or country.
  • Date of move.
Students studying out-of-province/country
  • Date leaving and expected date of return.
  • Name and address of school or university.
  • Duration of course.
  • Students studying outside of New Brunswick must provide proof of enrollment with the school or university on an annual basis.
Temporary absence from New Brunswick
  • Date leaving and expected date of return.
  • Destination.
  • Any absence over 182 days, whether for work purposes or vacation, would require the approval of the Director of Medicare.

*Identifying information includes name, Medicare number, date of birth, current address, and telephone numbers (work and home).

Replacement cards produced as a result of name changes, incorrect dates of birth, newborns or marriages which produce name changes or changes in expiry dates will be sent without charge.
