The issue

Older adults living with long-term illness are at risk of their physical and mental health getting worse. Many older adults do not use supports or resources in the community to live at home safely.

The importance

Many older adults have more than one long-term illness. Many older adults do not feel able to manage their long-term illness or symptoms. 

What makes this project different

We are offering a team-based, health care service within community health centers for older adults. The service includes several appointments over time with the health care team. To help the older adult manage their long-term illnesses and live safely at home. To help the older adult and their caregiver find and use community resources. 

Anticipated impacts

  • Older adults will be more able to take care or their long-term illness. They will report a better quality of life and feel safer at home.   
  • Older adults and caregivers will use community resources more to help stay safe and healthy in their home.  
  • Community health center teams will help older adults stay safe and healthy in their home.
