The issue

Getting ready to go home from the hospital can be hard. There’s a lot to plan and remember. Many people are involved. Your medications may have changed. It’s hard to take medication properly if you don’t know what changes were made or why. When information isn’t shared with your community pharmacist, they may not be able to help you. If you take medication that was stopped in the hospital, or if you have problems with medications, you may need to return to the hospital. We want to do a better job teaching seniors about their medication. We will work with seniors to make sure they have the tools and support they need to take medications safely at home. We will share information with community pharmacists so they can support seniors better.

The importance

Many seniors struggle to understand medication information. Community pharmacists may not have information they need to help. Older adults may have trouble with medications. You are more likely to have problems if you don’t understand how to take your medications. We can improve care by teaching seniors about their medications and sharing information with community pharmacists. We want to learn what seniors need and how pharmacists can help. Our goal is to prevent medication problems, improve health care, and save money.

What makes this project different

We will study how pharmacists can help seniors. We will teach seniors about their medications and share information. We want to find out what seniors need to take medications safely.

Anticipated impacts

This project will help seniors with the tools and support they need. We will try to solve medication issues before seniors go home. We will also talk with seniors’ community pharmacist. We believe that if seniors work with a hospital pharmacist you will have a better experience.  We think this will reduce medication problems and the need to return to hospital. We want pharmacists to be more involved in your care.
