A framework for collaboration and coordination that brings together youth-facing services in New Brunswick.

Overview

Integrated Service Delivery (ISD) is a way for people who help kids and teens—like teachers, nurses, counselors and social workers—to work together. 

It is a framework for service delivery designed to coordinate service and support for children and youth. 

This type of delivery is designed to keep people from having to repeat their story to new service providers and ensures those supporting a child or youth has the most accurate information, so they and their families get the help they need.

With Integrated Service Delivery:

  • children, youth, and families do not have to repeat their story multiple times, unless they choose 
  • professionals work together as a team to support each individual’s unique needs and goals
  • a broader range of supports and services can be identified through collaboration 
  • everyone works from a shared plan to provide consistent, coordinated support
