Integrated Service Delivery

A framework for collaboration and coordination that brings together youth-facing services in New Brunswick.

Overview

Integrated Service Delivery (ISD) is a way for people who help kids and teens—like teachers, nurses, counselors and social workers—to work together. 

It is a framework for service delivery designed to coordinate service and support for children and youth. 

This type of delivery is designed to keep people from having to repeat their story to new service providers and ensures those supporting a child or youth has the most accurate information, so they and their families get the help they need.

Formal support structure

Common Plan: Used when two+ departments are involved in supporting a young person to ensure continuity of care and information sharing.

Integrated Clinical Team (ICT): Made up of experienced clinical representation from youth serving government departments. This table reviews files that require higher tier intervention.

Regional Operations Committee (ROC): Designed to review regional operational and integration hurdles, regional training needs and high-level information sharing. The members of this committee although review PISC applications before they are submitted to that committee. 

Provincial Integrated Support Committee (PISC): Multi departmental table designed to review files that require funding beyond what is possible from any single department (includes adult services).

Indigenous inclusion 

First Nations Coordinators support the identification and resolution of systemic barriers, while supporting First Nations community services with advocacy, participation in community treatment planning and system navigation.
