Overview
You may apply for a birth certificate if:
- you are the person named on the birth certificate and are at least 16
- you are the parent(s) of a child under 16. Parent(s) must be listed on the birth record or provide court documents proving parentage
- you are the parent(s) of a child over 16 who is mentally incapable. Proof is required such as a court document or letter from a physician
- you have written authorization (consent) of the person named on the birth certificate
- you have been granted guardianship – proof is required
- you have a court order – proof is required
- you require it to comply with a specific act or regulation – proof is required.
- you are applying for the certificate of a deceased individual; proof of death is required
- the information required to obtain a birth certificate: name of individual (if married, maiden name), date of birth, place of birth, father's name, father’s place of birth, mother's full maiden name and given names, mother’s place of birth, relationship to person named on certificate, reason why certificate is requested, daytime phone number and return address
Birth certificates are available in short form or in a long form.
The short form includes the name of the individual, date of birth, place of birth, sex, registration date, registration number and date issued.
The long form includes the name of the individual, date of birth, place of birth, sex, registration date, registration number, date issued, plus the names and the province or country of birth of the parents.
Fees
The fee for a birth certificate is $45 when applying in person or by mail and $40 when applying online.
