Death certificate

The Vital Statistics Branch registers deaths and provides deaths certificates of deaths that have occurred in the province.

Overview

Any person may apply who had a relationship with the person named on the certificate and who provides a reason, the correct information and fee.

The information required to obtain a death certificate includes name of deceased, date of death, place of death, spouse's name, date of birth of deceased, name of funeral home (if known), relationship to person named on certificate, reason why certificate is required, daytime phone number and return address.

The death certificate includes name of the deceased, date of death, place of death, date of birth, province or country of birth of the deceased, registration number, registration date and date issued.
