Support for patients and their families through the care process.

Overview

The Cancer Patient Navigation Program is a service that supports patients and their families through the cancer care process, especially before diagnosis and the transition after treatment.

Cancer Patient Navigators are nurses with experience in oncology (cancer care) who work closely with patients and their families, the cancer care team and community resources to:

  • remain a constant source of support for you and your family
  • link you with support and other care professionals in your community
  • ensure you and your family and caregivers have all the information available about your cancer and treatment options
  • help you find resources to help with travel expenses during treatment
  • coordinate care with the health care team

This service is available through Horizon Health Network  and Vitalité Health Network . Cancer Patient Navigators can support you at any point during your cancer journey (diagnosis, treatment, follow up, or palliation etc.).
