Overview
The goal of the PharmaCheck program is to provide a face-to-face professional pharmacy service focused on the appropriate use of chronic medications to improve patient outcomes.
This can improve a patient’s knowledge of and adherence with their medications. This helps enhance the benefits and reduce the risks associated with their medications.
Eligibility
To be eligible for a medication check-up, a patient must be:
- a New Brunswick Prescription Drug Program (NBPDP) beneficiary covered under one of the following plans: Seniors (Plan A), Adults in Licensed Residential Facilities (Plan E), Clients of the Department of Social Development (Plan F)
- taking three or more medications for chronic conditions (e.g., diabetes, hypertension)
Only one medication check-up will be reimbursed per beneficiary per year (April 1 to March 31).

Claims submitted by participating providers must include the following information:

Documentation requirements
The patient must leave the face-to-face session with a complete and up-to-date personal medication record that includes the name and signature of both the patient and pharmacist who conducted the check-up, as well as the pharmacy’s contact information.
A copy of the signed record must be retained by the pharmacy for three years. After completing a medication check-up, it is expected that the patient will understand:
- the names of their medications
- why they are taking their medications
- the best way to take their medications
- any special instructions associated with the proper use of their medications
